Criminal justice agencies can access all of an individual's criminal history record information (CHRI). Requests made by noncriminal justice agencies and individuals are subject to edit criteria contained in the law.
Request for Criminal History Check. Criminal history background checks for individuals are available at the PATCH site or download form SP 4-164, print the completed form, and mail it. An individual background check is $22 and can be notarized for an additional $5.
