To obtain Harris Health financial assistance you must complete Harris Health’s “Application for Financial Assistance.” Be sure you, your spouse, and ALL children between 18 and 26 years old who live with you sign and date the form.

These patients are asked to mail a completed application for financial assistance two weeks before their temporary expiration date ends. Application packets and mailing address information can be found on the harrishealth.org website.
