Steps to access Classic Metalink Forums : Return to Classic Metalink from the My Oracle Support. (link at the left bottom of the page) or Login into the Classic Metalink from the login page. Submit a search with a generic work by choosing the "Technical Forums" from the drop down box in the "Quick Find" box.
Metalink note 437878.1calls for patch 7140392. Patch 7140392 actually turns out to be patch 7140405. (unzip of p7140392_10123_GENERIC.zip results in dir 7140405) While applying the patch 7140405 it is un-installed and re-installed, apparently 7140405 was already installed. During my analysis I was looking for 7140392.
