Recover Your MyChart Username. Your MyChart Username has been sent to the e-mail address on file with your account.If the information you submitted matched a MyChart account in our records, your username has been sent to the email address on file.
Follow these steps to sign up for a MyChart account. Enter your personal information. Verify your identity. Choose a username and password. If you have any questions, please contact us at (555) 555-5555.
If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 513-522-4278 (52-CHART) to help you regain access to your MyChart account.

To navigate to your vaccine record inside of MyChart: Login using your username or password. (If you don't have a MyChart account, activate it here.) Click Menu. Click COVID-19 (under the My Record heading). You should see a "COVID-19 vaccination complete" message in green.

Scheduled video visits are appointments with your healthcare provider where you connect via your patient portal: The Christ Hospital Mobile App or MyChart for Desktop. You will be able to see, hear and speak to your provider, similar to the experience you have with friends or family using Facetime.
